Carer Payment and Carer Allowance Medical reports for health care providers
These medical reports are used to assess eligibility for Carer Payment and Carer Allowance. They are not claim forms and can only be completed by a Treating Health Professional.
If you are a carer or nominated third party thinking of claiming Carer Payment and/or Carer Allowance, you should contact Centrelink on 13 2717 or visit a Centrelink office where our staff will ask some questions to determine individual circumstances and issue the appropriate forms.
From 1 July 2010 Carer Payment and Carer Allowance forms have changed. If you have previously provided a suitable medical report, you may not need to provide another one. You must contact Centrelink to find out if a medical report should be provided. You can do this by calling Centrelink on 13 2717.
- Carer Payment and Carer Allowance Medical Report (for a child under 16 years)
- Carer Payment and/or Carer Allowance Medical Report (for a person16 years or over)
This medical report is only to be used in circumstances where a personalised medical report is not able to be issued by Centrelink.
