Claim form
You can print out the claim form here or you can phone us on 13 2318 or visit your nearest Centrelink Customer Service Centre to have a claim form sent to you.
To help you complete the claim form, you should also read the information you need to know about your claim, located at the front of the claim form, including the privacy notice.
Lodging your claim
You can lodge your claim by posting or faxing your completed smartform or static form direct to one of our Assistance for Isolated Children Centres, or you can drop off your form at any Centrelink Customer Service Centre or Agent.

Information required to make a claim
You may be required to provide evidence to support your claim as well as details of your child's study. For more information, phone us on 13 2318.
Closing dates for claims
- The closing date for all Assistance for Isolated Children claim forms is 31 December in the year of study.
- Claims may be accepted if an intent to claim has been registered by 31 December of the year of study and the form lodged within 13 weeks. Claims will not be accepted under any circumstances if a form has not been lodged or an intent to claim has not been registered by 31 December of the year of study.
- You can register an intent to claim by contacting us by phone on 13 2318 or in person, post or by fax. If you register your intent to claim, you must lodge your claim form within 13 weeks of contacting us or by 31 December in the year of study (whichever is later) to be eligible for payment for that year.
