Carer Payment and Carer Allowance (caring for a child under 16 years) - claim forms and information

Note: You can make a claim for Carer Payment and/or Carer Allowance (caring for a child under 16 years) on the same claim form.

Intent to claim

You can register an intent to claim for Carer Payment and/or Carer Allowance. You should then submit your claim within 14 days. This will help you get paid from the earliest possible date.

If you are contacting on behalf of a family member or friend you can phone or contact us on their behalf to register an intent to claim.

You can only be paid from the date that you register your intent to claim or lodge your completed claim form.

Note: From 1 July claim forms and medical reports for Carer Payment and/or Carer Allowance will no longer be available from the Centrelink website. Instead, carers or a nominated third party will be required to contact Centrelink on 13 27 17 or visit a Centrelink office where our staff will ask some questions to determine individual circumstances and issue the appropriate personalised claim forms.

Claim forms to complete

Step 1

Contact Centrelink on 13 2717 or visit a Centrelink Customer Service Centre and our staff will ask you some questions to determine your circumstances and issue you with the appropriate claim forms.

Step 2

Read the information attached to your claim. This contains important information you need to know, including the privacy notice, before you lodge your claim for Carer Payment and/or Carer Allowance.

Returning your forms

Step 3

Return your completed forms, proof of identity for yourself, your partner (if you have one) and for the child or children being cared for, your medical report and any other documents we have asked for to your nearest Centrelink Customer Service Centre.

Note: To authorise a person to act on your behalf when dealing with Centrelink, or who can receive your payments, refer to someone to deal with Centrelink for you.
