Claim: To claim Maternity Immunisation Allowance, you can download the claim forms and information from this page, complete the forms and return them to your nearest Family Assistance Office. You can also call the Family Assistance Office on 13 6150 and ask to have a form posted to you, or you can pick up a claim form at any Family Assistance Office.

If you are having trouble downloading the forms and information, you can telephone Centrelink on 13 6150 or visit your nearest Family Assistance Office located in a Centrelink Customer Service Centres or Medicare Australia Offices.

Information required to make a claim

To choose a claim form that best suits your personal circumstances there are a few steps to follow:

Download the Family Assistance, The What, Why and How booklet as this contains important information about your claim that you must read before lodging your Newborn/Child Claim for Family Assistance and Medicare form.

Download and complete a claim form that best suits your own circumstances:

Step 3

Print and complete your downloaded claim forms with your personal details. You may need to download and complete some of the additional forms. Return your completed forms to your nearest Family Assistance Office.
