Intent to claim

You can register an intent to claim for Mobility Allowance by phoning us on 13 2717 or by visiting any Centrelink Customer Service Centre. This will ensure you are paid from the earliest possible date.

A family member or friend can phone us on your behalf to register an intent to claim.

Once you have registered your intent to claim, a Centrelink Customer Service Adviser will contact you within 2 days to assist you with your claim and send you a claim form.

Note: You can only be paid from the date that you register your intent to claim or lodge your completed claim form.

Claim forms to complete

The following steps will help you choose a claim form to suit your personal circumstances.

Step 1

Read the claim information form. This contains important information you need to know, including the privacy notice, before you lodge your claim for Mobility Allowance.

Step 2

Print out and complete a claim form that best suits your own circumstances.

If you have not previously claimed Mobility Allowance, you will need to complete a full claim for Mobility Allowance (including a Treating Doctor's Report).

  • Want to claim Mobility Allowance
  • Want to claim an Advance Payment of Mobility Allowance

Note: When you are making a new claim for Mobility Allowance, you can elect to receive the Advance Payment by completing the appropriate questions on the Claim for Mobility Allowance form. You do not need to complete this form as well.

Documentation

Step 3

You will need to provide proof of identity. If you have any questions, phone us on 13 2717 or visit your nearest Centrelink Customer Service Centre and a Customer Service Adviser will be able to help you.

Returning your forms

Step 4

Return your completed forms, proof of identity, treating doctor's report and any other documents we have asked for to your nearest Centrelink Customer Service Centre.

Note: To authorise a person to act on your behalf when dealing with Centrelink, or who can receive your payments, refer to someone to deal with Centrelink for you.
