Special Benefit claim forms and information
Intent to claim
You can register an intent to claim for Special Benefit. This will help you get paid from the earliest possible date.
A family member or friend can phone us on your behalf to register an intent to claim.
Note: You can only be paid from the date you register your intent to claim or lodge your completed claim form.

You may need to complete some additional forms as well as the claim for Special Benefit form. This will assist us to accurately assess your claim, based on your (and your partner's) personal circumstances.

Returning your forms
Step 3
Return your completed forms, proof of identity, proof of your income and assets, and any other documents we have asked for to your nearest Centrelink Customer Service Centre.
Note: To authorise a person to act on your behalf when dealing with Centrelink, or who can receive your payments, refer to someone to deal with Centrelink for you.
