Claim forms if you are ill, injured or have a disability:

If you are ill, injured or have a disability there are a number of payments you could be eligible to claim. Listed below are links to information and forms that may assist you in claiming a payment from Centrelink and /or the Family Assistance Office:

• Disability Support Pension - for people unable to work for 2 years due to illness, injury or disability.
• Sickness Allowance - for people temporarily unable to work due to illness, injury or disability.
• Mobility Allowance - for people undertaking paid work or training, who need assistance with transport.
• Newstart Allowance - for unemployed people aged over 21 OR people who are temporarily unable to work due to illness, injury or disability.
• Youth Allowance - for full-time students aged 16 to 24 OR unemployed people aged under 21.
