Help for health care providers completing Centrelink forms

People who are carers or people who have a disability, and who claim or receive a payment from Centrelink may also need to provide a medical certificate or treating doctor's report.

Many of these forms can only be completed by a medical practitioner. Medical practitioners can order these forms using Centrelink's forms and publications ordering service.

Practitioners are providing an essential service to their patients when they fill in these forms because they are required to support new claims and reviews for disability and carer payments.

Medicare Australia is expanding its Provider Enquiry line to support practitioners providing this service to their patients.
