• For Child Care Benefit as reduced fees, combined estimates for the current financial year*
• For Child Care Benefit for a past financial year, combined actual income for that year*
• For Family Tax Benefit in instalments, combined estimates for the current financial year*
• For Family Tax Benefits for a past financial year, combined actual income for that year
• For Baby Bonus, combined estimate for the 6 months following your child entering your care*
• For Parental Leave Pay, your own (not your partner's) estimated or actual income for the financial year prior to the date of your claim or the date the child entered your care, whichever is earlier

* Note that you do not need to provide your income details for Child Care Benefit, Family Tax Benefit or Baby Bonus until after your child enters your care

• Your passport number
• Details of periods you were outside Australia
• The date you first arrived in Australia if you were not born in Australia
• Your partner's details will also be required if you are claiming Family Tax Benefit

Details of periods your child has spent outside Australia

• Details of your work over the last 13 months, including hours worked and unpaid leave taken

• Your employer's Australian Business Number (ABN)
• Contact details for your employer

• Name
• Date of birth
• Details of any previous carer
• Details of shared care arrangements (Family Tax Benefit only)
• Details of child support received (Family Tax Benefit only)

• Not needed for Maternity Immunisation Allowance only

• The bank, building society or credit union account must be in your name. A joint account is acceptable. It cannot be in a child's name unless you are the signatory or trustee.
