When to claim
If you claimed Baby Bonus, the Family Assistance Office will automatically assess your eligibility for Maternity Immunisation Allowance.
If you did not claim Baby Bonus, you will need to claim Maternity Immunisation Allowance on or before your child’s 5th birthday.
For a child adopted from overseas
You will need to claim on or before your child’s 5th birthday or within 2 years of their arrival in Australia (whichever is later).
For a child who is stillborn or dies
You must claim on or before your child’s 5th birthday or within 2 years of the child’s death (whichever is later).

Claiming with forms
For a Maternity Immunisation Allowance claim form you can:
- print out and complete a claim for Maternity Immunisation Allowance form
- print out and complete a Claim for Bereavement Payment of Family Tax Benefit, Baby Bonus, Maternity Immunisation Allowance form
- phone us on 13 6150
- visit your nearest Family Assistance Office at Centrelink
Note: To authorise a person to act on your behalf when dealing with Centrelink, or who can receive your payments, refer to the someone to deal with Centrelink for you section of our website.

Documentation
When lodging your claim with us we may ask for extra information to assess your entitlement.
You will need to provide proof of birth or adoption of the child and any other verification documents requested within 14 days.
Claim outcome
When your child reaches the appropriate age for each amount, the Family Assistance Office will send you a letter advising if your claim is successful. If you are eligible for Maternity Immunisation Allowance, the letter will also tell you when you will be paid and how much you will get.
If you disagree with a decision we make
You always have the right to appeal any decision made by the Family Assistance Office.
