Medicare Card Number
This is the number of the Medicare card that your child appears on. If your child is not currently listed on a Medicare card, please provide the number of the Medicare card that you plan to add your child to.
The Medicare card number is in the format of NNNN NNNNN N.
Immunisation Status
There are different immunisation requirements depending on the payment type and the age of the child for whom you are claiming. For more information go to www.servicesaustralia.gov.au/immunisation.
For immunisation information:
- The Australian Childhood Immunisation Register can provide you with details of your child's current immunisation status and/or arrange to mail your child's Immunisation History Statement to you. The statement advises whether or not the child has met the immunisation requirements for Centrelink and shows the immunisations a child has had and when their next one is due.
- The Immunise Australia Program is run by the Department of Health and Ageing and provides general information about immunisation and exemptions. It cannot help with information particular to your own circumstances.
Child Care Benefit
All children under seven must meet immunisation requirements for Child Care Benefit. To meet the requirements, evidence must be available that either:
- the child has been fully immunised appropriate to the child's age
- the child is on a catch-up schedule
- an exemption from immunisation requirements is appropriate.
For a reduced fees claim for approved care, you have a 63 day immunisation grace period to meet the requirements. During this period you are eligible for Child Care Benefit by fee reduction for care provided by an approved child care service. If immunisation requirements have not been met after the 63 days have passed, Child Care Benefit and or Child Care Rebate is cancelled for the child until the requirements are met.
Medicare Reference Number
This is the single digit number listed beside your child's name on the your Medicare card. It can also be referred to as the individual reference number. It is in the format of N. (eg. 4) If your child is not currently listed on the Medicare card, please leave blank.
Family Tax Benefit
Children may be required to meet immunisation requirements for Family Tax Benefit Part A supplement for the years during which they turn one, two or five. To meet the requirements, evidence must be available that either:
- the child has been fully immunised appropriate to the child's age
- the child is on a catch-up schedule
- an exemption from immunisation requirements is appropriate.
To receive Family Tax Benefit Part A supplement for a relevant child, the child must meet immunisation requirements within 12 months from the end of the financial year in which the child turned one, two or five.
If you claim Family Tax Benefit as a lump sum and there are special circumstances that prevent the child from meeting immunisation requirements within the 12 month timeframe, please contact us to discuss.
